The company sponsoring DAMA South Florida in a one event Sponsorship understands that this form is binding when payment is submitted to the DAMA South Florida. Chapter. AGREEMENT WILL BE SENT WITH INFORMATION ON SINGLE EVENT.
DAMA South Florida accepts payment drawn on US banks, via electronic transfer of funds or via credit card. There is no refund on sponsorship if cancelled.
By submitting payment, I have read the terms and conditions included and have the authority to enter into this agreement. Sponsor agrees to abide by all requirements, restrictions, and obligations set forth in this agreement. Sponsor will provide a gift valued at a minimum of $100 to be given at event.
